mjpeg-player: added reloading of live playlists
Reduced level-contoller.js to keep only what we need (reloading of live playlists). Added a LevelController to Hls in hls.js. Removed stop message from player.js which was causing false stops. Minor changes to level-helper.js (formatting and logger).
